Co-Care (Collaborative Care for Polysubstance Use in Primary Care Settings) is a nationwide study that is testing a collaborative team approach to caring for patients with substance use.


ABOUT CO-CARE

  • Tests a collaborative care approach to caring for patients with substance use

  • Collaborative team includes provider, nurse, and health coach

  • Participants can receive $25 to $105 per month

  • You may be eligible if you use alcohol, opioids or painkillers, cocaine, other stimulants, or other substances

  • Nationwide study with 7 participating sites and various clinics (see map below)

  • Proudly led by a research team at New York University (NYU) Langone Health—however, NYU is NOT a participating site
